Our Services

Our Resident Care Program encompasses the following:

• Comprehensive resident health and needs assessment
  
• Creation of individualized resident care plans
  
• Ongoing monitoring of resident social, physical and cognitive well-being
  
• Ongoing Communication of resident well-being status and needs with family, physicians and Care specialists.
  
• Identify and schedule training opportunities for each Care Specialist based on the unique changing needs of each resident

The Care Program is managed by Dr. Caroline Bamu, the owner/Administrator. Caroline visits every prospective resident prior to moving in to initiate the assessment and care planning process. This includes working with resident families, physicians, skilled nurses and other healthcare providers to create ongoing individualized service plans and schedules that address the changing needs and preferences of each resident. Caroline also monitors the well-being of each resident regularly and provides supportive oversight to Care Specialists, families and primary care physicians.

Our Care Specialists provide the following services:

• Assistance with personal hygiene including bathing, grooming and dressing
  
• Nutritious meals three times a day, plus healthy snacks
  
• Family-style dining and assistance with self-feeding
  
• Assistance with hydration
  
• Medication administration assistance
  
• Incontinence care
  
• Housekeeping and laundry services
  
• Hospice care planning and end of life assistance
  

Personal Care Assistance is provided by our staff of certified, trained and compassionate Care Specialists 24 hours a day, 7 days a week. We work with our residents, their families, primary care physicians and dietitians to identify specific resident needs on a regular basis.

A common reason for admission into assisted living facilities is to receive assistance with medication management. At Cardan Manor, our Owner/Administrator is a Registered Pharmacist and Pharm D, licensed with the State of Colorado. 

We provide the following services to improve medication therapy outcomes for each resident:

• Monthly drug regiment review
  
• Review medication orders for accuracy, contraindications, drug/drug interactions and allergies
  
• Medication reconciliation & recommendations to primary care physicians
  
• Arrangement for refills and pickups
  

Our Medication Therapy Management service helps to prevent medication mismanagement while promoting resident health, functionality and independence by decreasing occurrences of hospitalizations and physician visits originating from medication-related adverse events or polypharmacy

We organize and supervise the following activities based on resident interests and needs, with a goal of providing structure and routine needed to cultivate feelings of security and certainty in the day-to-day life:

• Gardening, Arts and craft
  
• Cooking and Baking
  
• Music time and sing-alongs
  
• Excursions to sporting events, movies, the library, park, zoo or museum
